Car only, no mattress or slats. However, we never needed slats. Just put a twin mattress with or without a box springs, and they're good to go. The slats only complicate it. The hood opens up for a large toy box area, and the tail of the car makes a nice book shelf, or whatever. The car is just about indestructible, has a few scratches, but looks great.
